Steps to start an appeal here:
Create an account on this website.
Verify your mail address via the email sent over the mail.
Link your village and verify via in-game API token.
Start a discussion over here.
Choose the tags should be set at Ban Appeal.
Format of making appeal post should be:
Title: #Player_tag
In Major Text:
IN-GAME NAME(#player_tag)
Reason Of Ban
Why should your ban be removed
The alternative accounts(if any)
Note: If you own several villages and want to appeal for each of them. Please link all the villages and verify each of them via API token as told in step 3rd.
